Case Twenty One - Splenic Sarcoidosis

Clinical History: 31-year-old black female with history of pulmonary sarcoidosis presents with complaints of left sided abdominal pain and early satiety. Physical exam reveals splenomegaly.

Findings: CT of abdomen and pelvis with IV and oral contrast demonstrates massive, heterogeneous spleen. Abnormal soft tissue density is seen in the porta hepatis and retroperitoneum consistent with lymphadenopathy.

Diagnosis: Splenic sarcoidosis (pathologically proven following splenectomy).

Discussion: Although marked abdominal CT findings are uncommon in patients with sarcoidosis, characteristic abnormalities include lymphadenopathy in addition to hepatic and splenic enlargement. These findings correlate with disease activity but not with chest radiographic stage.
